  • Benefit checks and filling in benefit forms
  • Housing issues
  • Legal matters
  • Funeral directors
  • Applying for a blue badge
  • Signposting to local authority and other relevant agencies
  • Advocacy and letter writing on the patients behalf
Supporting Patients prior to their discharge from st john’s

Prior to discharge the role of the social worker works alongside the patient, their family and carers and other hospice professionals to facilitate a safe discharge, should it be in your home or a residential or nursing establishment.

Psychological and Emotional Support

When someone close to you has a serious illness, for some individuals the diagnosis can be painful and difficult to deal with. We can offer the patient and their family emotional and psychological support, by providing an opportunity to talk about your concerns and ensuring you are provided with support in a confidential setting.
