Here is how to use our online dedication form:

Select how many dedications you would like to make from the drop down option below.
Fill in the names of your dedications into the boxes and select the donation amount you would like to make for each (You can also add an additional donation at checkout).
Add any messages for the online photo wall and upload photos of your dedications for online photo wall – both entirely optional.
Please use jpg or png files, no larger than 5mb. Please note larger files may take a moment to upload.
Once complete click Add to Cart and then check out by clicking ‘View Cart’. You can then fill in your own details and pay through our online shop.
