Here is how to use our online dedication form:

• Select how many dedications you would like to make from the menu.
• Add the names of your dedications to the boxes, then select your donation amounts using the sliders (You can also add an additional donation at checkout).
• Use the message and add photo boxes if you would like to add your dedications to the online photo wall – this is entirely optional.
  Please use jpg or png files, no larger than 5mb. These will be uploaded when you add your dedication to the basket.
• You can also select how many limited-edition Tree Ornaments and Pin Badges you’d like.
• Once complete click Add to Basket and then check out and pay by clicking ‘View Cart’.
